Well I don't think whales would do that because I don't think they'll be smart enough (No offense whales).
However if they had fingers since many thousands of years ago and had evolved into a more advanced creature then I think they'll be fine and won't need hospitals.

I have no idea why I answered this question, I just felt if I didnt nobody would.
